When filing for Decease Account below are the requirement:
1. Death Certificate (Original and Photo Copy).
2. Extra Judicial Settlement form (Attorney) - Cost it defends on the amount you will claim. It cost us Php3,000.00 + Cedula + SPA(Special Power of Attorney) Php500.00.
3. Indemnity Agreement (Bank form) - if Php300,000.00 up is the amount you will claim; required 2 to 3 weeks publishing.
4. Photo Copy 2 valid ID.

Today I am contacting PhilPlans Las Pinas to know the requirement in claiming the Educational Maturity Benefit for my Daughter.
Below are reminders on claiming your check benefits:

Submit the following requirements from January 18 up to February 23, 2013 to any of PhilPlans Branch Offices.
1. Original AND photocopy of Plan Contract
2. Original AND photocopy of Certificate of Full Payment
3. Original AND photocopy of two (2) valid IDs bearing your photo and signature (i.e. Driver’s License, Passport, SSS, GSIS, PRC ID)
4. Three (3) copies of fully-accomplished Benefit Settlement Form. You may download the form at this link.
Important: Please indicate in the Benefit Settlement Form your preferred Option:
1. Option 1: LUMP SUM
2. Option 2: PERIODIC/SEMESTRAL RELEASE
If the documents were received after February 23, 2013, the date of check release shall be April 30, 2013.
NOTE: For lost CFP and Policy Contract, planholders must present the following:
1. Notarized Affidavit of Loss
2. Photocopy of two (2) valid IDs bearing your photo and signature
3. Php 300 processing fee
