1. Application Form
2. TCT of deceases original and 1 photocopy
3. Valid ID with (3) specimen signature ng heirs.
4. Affidavit of surviving heirs consenting to the Maynilad installation (Certified True Copy)
5. Barangay Certificate/Clearance(Original) that will specify that the purpose is for Maynilad Installation.
6. Utility Bills
