6. Proceed sa cashier. Magbayad ka.

7. After bayad will give to piece of paper balik sa ER billing.

8. ER billing will check ang papupuntahin ka sa cashier sa may billing sa labas ng ER bibigyan ka ng release slip.

9. Then punta kana sa morgue at tawagan mo na ang funeraria st peter.

10. Death certificate need mo fill up yung form. Then aayusin nila. Pag okay na pirmahan nila. Then will give instruction. Pa pirmahan sa health center ng dasma sa may guevarra st. But need muna mapirmahan ng embalsamador. Then health center then sa munisipyo rehistro for death cert registry no.
